ข้าพเจ้าตกลงยินยอมตามเงื่อนไขในใบสมัครนี้ ดังนี้ 
I hereby covenant to comply with the conditions contained in this application from, as follows:

1. หากบริษัทฯ ตกลงรับข้าพเจ้าเข้าทำงานแล้ว ข้าพเจ้าตกลงให้งานที่เกี่ยวกับงานสร้างสรรค์ วรรณกรรม โปรแกรมคอมพิวเตอร์ นาฏกรรม ศิลปกรรม ดนตรีกรรมโสตทัศนวัสดุ ภาพยนตร์ สิ่งบันทึกเสียง งานแพร่เสียงและแพร่ภาพ ซึ่งข้าพเจ้าได้ก่อหรือทำขึ้น นับตั้งแต่วันที่ข้าพเจ้าเริ่มทำงานกับบริษัทฯเป็นต้นไป จนถึงวันที่ข้าพเจ้าลาออกหรือพ้นสภาพจากการทำงานกับบริษัทฯ ในภายหน้านั้น ให้ลิขสิทธิ์ สิทธิและหรือกรรมสิทธิ์ของงานดังกล่าว ตกหรือโอนเป็นของบริษัทฯ แต่เพียงฝ่ายเดียว โดย ข้าพเจ้าขอสละสิทธิ์ และไม่ติดใจเรียกร้องค่าลิขสิทธิ์ค่าชดเชย และหรือค่าเสียหายใดๆ จากบริษัทฯ ตามพระราชบัญญัติลิขสิทธิ์ทีมีผลใช้บังคับในขณะนี้และหรือที่จะมีผลใช้บังคับต่อไปในภายหน้า ตลอดจนกฎหมายอื่นใดที่เกี่ยวข้องอีกต่อไปทั้งสิ้น If employed by the Company, I agree that all the copyrights, rights and/or ownership of works of authorship, literature, computer programs, drama works, audiovisual works, cinematographic works, sound recording, and sound and video broadcasting works, which I may have made or created from the day on which I start working with the company to the day on which I resign, or on which my employment with the company is terminated, shall be vested entirely in or transferred to the company. I agree to relinQuish my rights of or claims for any royalties for copyrights, compensation and/or damages from the Company by virtue of the Copyright Act, or any other relevant laws currently and/or subsequently in force.
2. ในกรณีที่ข้าพเจ้าผิดสัญญา หรือไม่ปฏิบัติตามข้อกำหนดเงื่อนไขที่กล่าวมาในข้อ 1 ข้างต้น บริษัทฯ มีสิทธิ์ดำเนินการใด ๆ เพื่อรักษาปกป้อง คุ้มครองและเรียกร้อง ลิขสิทธิ์ สิทธิ และหรือกรรมสิทธิ์ในงานดังกล่าวของบริษัทฯ ตามกฎหมายได้ทุกประการ ตลอดจนมีสิทธิเรียกร้องค่าเสียหายที่เกิดขึ้นเป็นอีกส่วนหนึ่งต่างหากได้ด้วย In an event where I am in breach of, or fail to comply with, the conditions set forth in 1 above, the Company shall, in accordance with the laws, have the right to take any action in order to maintain, safeguard, protect and claim for the copyrights, rights and/or ownership of such works, inclusion the right to demand compensation for damages therefore.
3. ข้าพเจ้ายินยอมให้บริษัทฯ เก็บ รวบรวม ใช้ เปิดเผยข้อมูลส่วนบุคคลและข้อมูลที่มีความอ่อนไหวของข้าพเจ้า (ถ้ามี) ให้กับผู้ประมวลผลของบริษัทฯ (ถ้ามี) รวมถึงตรวจสอบไปยังหน่วยงานของรัฐ หรือหน่วยงานอื่นที่เกี่ยวข้องกับข้อมูลส่วนบุคคลและ/หรือข้อมูลที่มีความอ่อนไหวของข้าพเจ้า (ถ้ามี) เพื่อการดำเนินการตามกระบวนการสมัครงานและการ พิจารณาการว่าจ้างทำงาน ข้าพเจ้ารับทราบว่า กรณีที่ข้าพเจ้าได้รับการว่าจ้างจากบริษัทฯ บริษัทฯ จะทำการเก็บ รวบรวม ใช้ข้อมูลบนใบสมัครนี้จนกว่าสัญญาว่าจ้างของข้าพเจ้าหรืออายุความการดำเนินคดีจะสิ้นสุดลง แล้วแต่ระยะเวลาใดสิ้นสุดลงในภายหลัง แต่หากไม่มีการว่าจ้างเกิดขึ้น บริษัทฯ จะทำการเก็บ รวบรวม ใช้ข้อมูลบนใบสมัครนี้เป็นระยะเวลา ..10... ปี นับจากวันที่ข้าพเจ้าลงนามในใบสมัครฉบับนี้ I hereby consent to the Company to collect, use, disclose to the Company's Data Processor (if any) of my personal data and sensitive personal data (if any), and also conduct the inspection to governmental organizations or other regulartory bodies which relates to the personal data or sensitive personal data (if any) for the purposes of the recruitment process and the consideration for employment. I hereby acknowledge the event that I am employed by the Company, the Company will collect and use the information provided on this Employment Application until the termination of my Employment Contract or the period of prescription for prosecution is expired, whichever is longer. In case there is no employment, the Company will collect and use the information provided on this Employment Application for a period of ...10.. years) commencing from the date hereof.

ข้าพเจ้าขอรับรองและให้สัญญาว่าข้อความที่ข้าพเจ้ากรอกในใบสมัครนี้ เป็นความจริงและสมบูรณ์ทุกประการ การบิดเบือนความจริง การแจ้งเท็จ หรือการปิดบังข้อเท็จจริง จะเป็นสาเหตุอันเพียงพอที่จะปลดข้าพเจ้าออกจากงานได้ ข้าพเจ้ายินดีให้สิทธิ์บริษัทฯ ในการสอบถามรายละเอียดต่างๆ และติดต่ออดีตนายจ้างหรือผู้ที่ข้าพเจ้าอ้างอิงไว้ในใบสมัครนี้ได้โดยมิต้องบอกกล่าวล่วงหน้าเพื่อขออนุญาตจากข้าพเจ้าก่อน และหากบริษัทฯ รับข้าพเจ้าเข้าทำงาน ข้าพเจ้าตกลงยินยอมปฏิบัติตามข้อกำหนด เงื่อนไข หรือข้อความใดๆ ที่ระบุไว้ในใบสมัครนี้ รวมตลอดจนข้อบังคับการทำงาน ข้อพึงปฏิบัติของพนักงาน และระเบียบต่าง ๆ ของบริษัทฯ ทั้งสิ้น ทั้งนี้ ข้าพเจ้าได้อ่านและรับทราบ "คำชี้แจงเกี่ยวกับข้อมูลส่วนบุคคลของอลิอันซ์ อยุธยา สำหรับผู้สมัครงาน" (ที่แนบมานี้) หรือ (insert QR code)
